1. Painless Ulcers: The hallmark symptom of granuloma inguinale is the development of painless ulcers or sores in the genital area. These ulcers typically start as small, raised, beefy-red bumps that gradually enlarge and erode, forming deep, irregularly shaped ulcers. The ulcers may have a granular or "beefy" appearance and can bleed easily. They are usually painless, which can delay diagnosis and treatment.



2. Slow Progression: The ulcers associated with granuloma inguinale tend to progress slowly and may persist for weeks or months if left untreated. The infection can gradually spread to nearby tissues, causing further destruction and enlargement of the ulcers. Without appropriate treatment, the ulcers can become extensive and lead to significant tissue damage.



3. Inguinal Lymph Node Enlargement: As the infection progresses, the nearby lymph nodes in the groin area (inguinal lymph nodes) may become enlarged and tender. This is known as lymphadenopathy and is a common feature of granuloma inguinale. The affected lymph nodes may feel firm and rubbery to the touch.



4. Absence of Pain or Itching: Unlike many other sexually transmitted infections, granuloma inguinale typically does not cause pain or itching. This lack of symptoms can make it challenging to recognize and diagnose the infection, leading to delayed treatment.



5. Secondary Bacterial Infections: Due to the open ulcers and tissue damage caused by granuloma inguinale, secondary bacterial infections can occur. These infections can cause additional symptoms such as increased pain, pus discharge, and foul odor from the affected area.



6. Systemic Symptoms: In rare cases, individuals with advanced or untreated granuloma inguinale may experience systemic symptoms such as fever, malaise, and weight loss. These symptoms indicate a more severe infection and require immediate medical attention.



7. Complications: If left untreated, granuloma inguinale can lead to serious complications. The infection can cause extensive tissue destruction, leading to disfigurement and scarring of the genital area. In severe cases, the infection can spread to other parts of the body, including the bones and internal organs.
