It is a chronic disease. If I had not had a type of surgery I would have been in a wheelchair or bed bound by now. Progression is not a set time frame each person is different. Some Cysters have been dealing with no surgery for 10 years or more. Mine progressed fast, within 10 months I was losing body functions. If I had had caring doctor's from the start I might could have slowed the progression with a diet/lifestyle change. Stay away from Physical therapy, it expedited the progression. Water therapy is the way to go.
Can it get better? I personally do not believe it.

Once you have surgical intervention you can start to see improvements in many ways. Although not all patients are in the same situation so we should not expect outcomes to be the same as others with thus disease

Tarlov is chronic and progressive disease. It rarely gets better

If the cysts are symptomatic it is a chronic condition that can progress.

Symptomatische Tarlov-cyste ziekte is een progressieve aandoening waarbij uiteindelijk de dag zo veel mogelijk liggend doorgebracht moet worden om zo min mogelijk pijn/ongemak te ervaren. Sommigen zijn een tijdje verlost van de pijn/ongemak indien de Tarlov-cyste operabel is echter, op een andere plek kan na verloop van tijd weer een Tarlov-cyste ontstaan.

Symptomatic Tarlov Cysts cause pressure on the surrounding nerves and over time can do irreparable damage to those nerves. This makes the prognosis very dim for those people who go undiagnosed for years. Surgical treatment has a far better outcome when done early.

They are not life threatening but they can cause chronic pain very severe
